Clinical Case - Leprosy Disease, Neurogenic Ulcers

LEPROSY DISEASE
NEUROGENIC ULCERS
 

 

CLINICAL MANIFESTATION

The leprosy can present several cutaneous manifestations, as lesions: 
- plane or maculares with uniform infiltration, 
- bounded or diffuse, 
- diffuse and infiltration non uniform with formation of small nodules and plates, 
- eritema, 
- oedema, 
- brown coloration or ferruginosa.  

Generally
1. lesion macular indicates initial phase of the disease, 
2. bounded lesion points of lesions tuberculoide, 
3. diffuse lesion is indicative of lesion virchowiana, 
4. bounded lesion and diffuse it characterizes lesion dsmorfic, 
5. coloration ferruginosa indicates the aspect virchowiana or dimorfo-virchowiana. 

The treatment clinical classic of the pathology should be made. 

TOPICAL TREATMENT

The cutaneous lesions can present secondary complications, as local infection. 

In the topical treatment of these lesions it can be used several products, can be used: silver
sulfadiazine with cerio nitrate
, enzymatic ointment (collagenase, fibrolynosin), hydrocoloid, hydrogel, linoleic acid, ricinoleic a
cid, bioperoxoil, Aloe vera, elicine, papain, albumin, non adherent gauzes, calcium and
sodium alginate
etc. 
The change period will depend on the chosen product.

 

CLINICAL CASE

F.S., 46 years, male. Leprosy virchowiana, with chronic ulceration to 12 years in external maléolo of the left leg. Dimension 13x5 cm. The bed of the lesion possessed aspect fibrinoso (Photo 1).

Treated ointment enzymatic collagenase for 7 days and successively with silver sulfadiazina
and cerio nitrate
for other week. It developed with reduction of the lesion and discreet formation of the granulation tissue (Photo 2).
