Clinical Case - Cutaneous Zooparasitosis

ZOOPARASITOSIS DISEASE
DERMATOLOGICAL WOUND

 

MIIASIS FURUNCULOIDE

Zooparasitosis - miiasis, is caused by the active penetration of  Dermatobia hominis larva in the normal skin or preexisting wounds, as, for example, refractory chronic ulceration.

The larvae of some species of flies, as the Phormia regina (blackbottle blowfly), is fed of necrotic tissue.

Zooparasitosis can denote the partner-economic and cultural level of determined  country or community.

Injury characteristics of the formed for the zooparasita:
1. nodular superficial formation during the larval development;
2. reached the larva maturity, the nodule comes out, leaving door of entrance to the secondary infection.

CHARACTERISTICS and
TOPICAL TREATMENT

The treatment of miiasis consists of the withdrawal of each maggot of manual form or by means of others techniques.

The preexisting cutaneous injury will have to be treated adequately.

Some topical products, such could be used as: sulfadiazine with cerio
nitrate
, enzymatic ointment (collagenase, fibrolynosin), hydrogel, linoleic acid, ricinoleic
a
cid, bioperoxoil, papain, calcium
and sodium alginate
.

 

 

 

CLINICAL CASE

F.S., 44 years, male, chronic osteomielite of tibia has 11 years, with ample chronic ulcer and bone exposition in the previous region of the leg - dimension of 16x3cm (Photo 1). Miiasis furunculoide in the injury presents has two months (Photo 2).

The withdrawal of the maggots in some stages and dressings with linoleic acid (essential fatty acid) had been carried through, substituted to each 12 hours.

The patient was guided how much to the prevention of zooparasitosis.

The resolutive behavior for this in case that it could be an ample debridament of the necrotic bone tissue and microsurgical reconstruction, by means of the free transference of remnant to muscle, associated or not to the microsurgical transference of bone remnant. The patient, however, did not congregate clinical conditions for the cited procedure.

This case was been about interdisciplinary approach, in the Clinical Hospital of the State University of Campinas (San Paulo - Brazil) by Reconstructive Surgery and Microsurgery Unit - Departament of Orthopedia and Traumatology (then chief L.C.Candido, MD, PhD) and Hyperbaric Medicin Unit (then chief P.E. Iazzetti, MD, PhD).

Biological Debridement

Is about biological debridement, carried through for barren maggots of flies prepared in laboratory. The larvae of some species of flies, as the Phormia
regina
(blackbottle blowfly), are used in the treatment of wounds, therefore they are induced to feed itself of the necrotic tissue convenientid.
